What’s Changed with Medicare from 1 July 2025?
If you’ve been managing a chronic condition like diabetes, heart disease, or PCOS—and you’ve been seeing or would like to see a dietitian under a Chronic Disease Management Plan through Medicare—then you might have heard some changes from 1 July 2025.
Don’t worry! Here’s what it all means and what you need to do next.
A new name
Before July 2025, your GP probably set up a GP Management Plan (GPMP) and something called a Team Care Arrangement (TCA) to help coordinate your care.
As of 1 July, those two plans have been merged into one: it’s now called a GP Chronic Condition Management Plan (or GPCCMP).
It still covers the same kind of support—like referrals to a dietitian or physiotherapist—but it’s simpler, faster to set up, and easier for everyone to follow.
More regular reviews
Whether your doctor creates or reviews your plan, the rebate is now the same (around $156.55 for GPs). This helps encourage regular reviews, which is excellent for your ongoing care. No extra costs for you—just a more streamlined process.
You no longer need a “Team” to get dietetic support
Under the old system, to get Medicare-covered allied health services (like a dietitian), your GP had to coordinate with at least two other healthcare providers.
Now that step has been removed. All you need is your GPCCMP and a referral letter from your GP, and you’re good to go.
So if you’re thinking, “I just need help with my eating habit to manage my diabetes,” you can go straight to a dietitian (at The Healthy Eating Clinic) with a referral letter—no complicated team setup needed.
What this means for dietitian visits
Under the new plan, you’re still eligible for:
- Up to 5 individual allied health visits annually through Medicare (dietitians, physios, etc.)
- These are still rebated under Medicare. For a dietetic services at The Healthy Eating Clinic Medicare will rebate $61.80 (Item 10954).
Important: For the referral to be valid, you must have had your plan prepared or reviewed in the last 18 months.
Already had a plan before July 2025?
Good news! If you already had a GPMP or TCA before 1 July 2025:
- That plan stays valid until it’s expiry, 12 months after the referral date.
- You can keep using your existing referrals until you use up the sessions or the time runs out.
- But if you need a new plan or a review now, it’ll be done under the new GPCCMP system.
What should you do now?
